Exhaled nitric oxide and inhaled corticosteroid dose reduction in asthma: a cohort study.

چکیده
Inhaled corticosteroids (ICS) reduce airway inflammation; however, guidelines recommend titrating ICS dose based on symptoms [1], which are not closely associated with airway inflammation [2]. Once symptoms are controlled for o3 months, ICS reduction is recommended [3] but step-down is often not implemented. Studies suggest that the majority of patients treated with ICS can have their therapy stepped down, although there is no clear evidence on how best to achieve this [4].

Tailored interventions based on exhaled nitric oxide versus clinical symptoms for asthma in children and adults.
BACKGROUND The measurement of severity and control of asthma in both children and adults can be based on subjective or objective measures. It has been advocated that fractional exhaled nitric oxide (FeNO) can be used to monitor airway inflammation as it correlates with some markers of asthma. Interventions for asthma therapies have been traditionally based on symptoms and/or spirometry. OBJEC...
متن کاملThe use of exhaled nitric oxide to guide asthma management: a randomized controlled trial.
RATIONALE Current asthma guidelines recommend adjusting antiinflammatory treatment on the basis of the results of lung function tests and symptom assessment, neither of which are closely associated with airway inflammation. OBJECTIVES We tested the hypothesis that titrating corticosteroid dose using the concentration of exhaled nitric oxide in exhaled breath (Fe(NO)) results in fewer asthma e...
متن کاملExhaled nitric oxide and asthma control: a longitudinal study in unselected patients.
Controlled studies have shown that monitoring of the exhaled nitric oxide fraction (F(eNO)) improves asthma management. However, the studies seldom consider the full range of patients seen in clinical practise. In the present study, the ability of F(eNO) to reflect asthma control over time is investigated in a regular clinical setting, and meaningful F(eNO) cut-off points and changes are identi...

Whether exhaled NO helps to identify a specific phenotype of asthmatic patients remains debated. Our aims were to evaluate whether exhaled NO (FENO(0.05)) is independently associated (1) with underlying pathophysiological characteristics of asthma such as airway tone (bronchodilator response) and airway inflammation (inhaled corticosteroid [ICS]-dependant inflammation), and (2) with clinical ph...
متن کاملChanges in the dose of inhaled steroid affect exhaled nitric oxide levels in asthmatic patients.
An increased concentration of nitric oxide (NO) in the exhaled air of asthmatic patients may reflect inflammation of the airways, and exhaled NO may, therefore, be useful in monitoring asthma control and the optimal use of anti-inflammatory treatment. We have studied the effect of reducing and then increasing the dose of inhaled steroid on exhaled NO, lung function and symptoms in 14 asthmatic ...
